CSC ID Renewal

Time to renew? 

To renew Community Service Chaplain Certification you must be actively involved in a community service ministry through an agency, institution, or local church, AND pass a new criminal background check.

ID Certification must be updated every two years. You can do so easily online here or through mail.

Renewing your ID online is easy!


Simply email the following to CSC@cogchaplains.org with the subject "Online ID Renewal" :


1.  A completed ID Renewal Form, located below

2.  A photo of old ID and

3.  A current head shot

4.  Pay online renewal fee through PayPal link below

To renew by mail, simply submit the following:

1.  A completed ID Renewal Form (located below) 

2.  A check or money order for renewal fee ($60) OR pay through PayPal link below

3.  A copy of your old ID

4.  A current photo (head shot only), either:

       a.  A digital photo emailed to CSC@cogchaplains.org

       b.  Hard copy photo with Renewal Form

"It's been more than two years since my ID expired. Can I still renew?"

If you're still actively serving or would like to resume service, you can renew your ID using the same steps above. Once we receive a renewal form with your pastor/supervisor's signature and other items listed above, your status will be updated. We will send you a new ID, and you will once again be recognized as an active COG Chaplain!
